We provide you access to over 975,000 participating providers through PHCS Practitioner and Ancillary PPO Network, including physician and ancillary providers for outpatient care stateside. Before scheduling appointments or receiving services we recommend that you visit:

for in-network providers.
Note: Call provider to confirm that is an in‐network provider for PHCS Practitioner and Ancillary PPO Network. If provider is out of network, services will not be eligible for payment, and you will be responsible for the full amount.
Need Elective Inpatient Care or an Inpatient Surgical Procedure and want to go Stateside?
1. Call SmartDecision Concierge 1.844.464.4277 Option 5.
Our Concierge will coordinate the services with the facility providers in-network you need.
It is very important that you do not coordinate services with providers without contacting
SmartDecision Concierge first. Once you receive confirmation from your Concierge Advisor,
schedule an appointment with the selected provider and get the care you need.
2. Visit www.multiplan.com/webcenter/portal/ProviderSearch
to see if the provider is in-network.
If the provider is out of network, services will not be eligible for payment, and
you will be responsible for the full amount.
